Any Equitpment that drops from monsters have a chance to have a random rarity assigned to it.
You can also randomly add rarity to your items through scrolls obtained through Item Mall/Blue Chip shop/Secret Room shop.
There is a total of 7 types of rarity:

Old rarity is gained from "Old Scroll of [stat]" found in treasure chests and red chip shop which will give you upto +4 and that stat the scroll gives.
